Pre-Participation Physical Exams - August 2nd

Athletic Pre-Participation Physical Exams

for NEW TO SOUTHWESTERN COLLEGE student-athletes will be held on:

Thursday, Aug. 2 beginning at 2 pm in DeVore Stadium

All sports student-athletes are welcome to participate, although a second date will be scheduled for teams starting later.

You must be 18 years of age or bring parental consent signed with your completed forms.

Returning SWC student-athletes do not need to attend this date unless they have an issue they would like to discuss with the team physician.

Bring or wear:

PEN

SHORTS & ATHLETIC SHIRT

INSURANCE CARD-photocopy of front and back

COMPLETED FORMS-if possible

IF UNDER 18, SIGNED PARENTAL CONSENT FORM WITH OTHER FORMS COMPLETED & SIGNED BY PARENT

Placement in line preference will be given to: 1. Students with parent in attendance, 2. Military Veterans and Active Duty Military with ID
