Pre-Participation Physical Exams (PPE's)

All sports are welcome - Fall, Winter & Spring
Thursday, August 4th 3:00 pm
DeVore Stadium (Under the Press Box in the shaded area)

If you cannot make the August 4th date there will be a make-up date on Thursday, August 11th @ 3:00 pm.

If you are a RETURNING SWC student-athlete, you do not need to complete the PPE again, but you must come to the Athletic Training Clinic at DeVore Stadium to restore your previous PPE and complete your returner’s packet. 

You must have an approved PPE or returner packet completed & on file with the athletic training staff before you can participate in your first practice with your team.  

PPE’s will begin with paperwork at 3:00pm on the west side of the stadium, in the shade beneath the press box. 

Bring:  Pen, Insurance Card Photocopy that can be turned in (please enlarge to 120%), Loose fitting clothing/shorts & your patience.  There are several stations to be completed, and lines will develop. 

If you are under 18 years, you need to have parent present for the PPE.
